Occupational TherapistRequirements:Bachelor of Science Degree in Occupational TherapyState LicensureSupervision:Occupational Therapist will report directly to the Program Manager who is responsible for overseeing direct clinical skills, documentation skills, and compliance to all professional standards. A review of performance will be conducted on a yearly basis.Job Description:The OTR is responsible for the evaluation and treatment of residents who demonstrate disorders or neurological deficit, weakness, and paralysis. Evaluation and treatment will only be conducted after written order is received from the physician.The OTR is responsible for the screening of residents of the facility as follows:New Patients: Within 24 hrs. of admissionCurrent Patients: Upon receipt of a "significant change" formAnd/ or upon referral from nursing.The OTR is responsible for the completion of all necessary documentation, which includes but is not limited to the following items:Initial Evaluation FormsWeekly progress notesMonthly Recertification formDischarge SummaryAll other required forms as designated by the facility and ManagerThe OTR is responsible for attending all required facility meetings and in services, including if need be Medicare meetings be, Patient staffing, family meetings, and care plans.The OTR will in cooperation with the program manager develop programs appropriate to the needs of the residents in the facility, to assist them achieving their highest feasible level of functioning.The OTR will assist the facility in meeting the standards necessary to obtain state, federal and other appropriate certification.The OTR will employ and follow all appropriate facility infection control policies and procedures and will respect and follow OBRA Residents rights guidelines.The OTR is responsible for the scheduling and completing of in-services for facility and staff educationThe OTR will represent her/himself in a professional manner at all times including professional conduct, professional communication and appropriate dress while in the facilityThe OTR is responsible for other duties assigned by the Therapy Manager/Director and facility.**The OTR is responsible for traveling between both hospital locations - Pontiac and Garden City
