PRN Occupational Therapist (OT) Aventura at Humility House | Austintown, OH Aventura at Humility House is seeking a compassionate and dedicated PRN Occupational Therapist (OT) to join our in‑house rehabilitation team. This flexible opportunity is ideal for a clinician who enjoys working in a Skilled Nursing Facility/Long‑Term Care (SNF/LTC) environment and is committed to helping residents achieve their highest level of independence and function. Responsibilities: Evaluate and treat residents in accordance with physician orders and established care plans. Develop individualized treatment programs focused on maximizing independence and quality of life. Complete timely and accurate documentation utilizing Net Health and PointClickCare (PCC) . Collaborate with therapy, nursing, and interdisciplinary team members to ensure exceptional patient outcomes. Maintain compliance with all facility, state, and federal regulations. Qualifications: Current Ohio Occupational Therapist license in good standing. SNF/LTC experience preferred, but not required. Experience with Net Health and PointClickCare (PCC) preferred.
