Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist opportunity in Tillamook, OR where you will help patients recover from injuries and manage chronic conditions while working in a hospital-based home health setting. You will plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions while serving as a co-leader in an interdisciplinary approach to patient groups.

Minimum Qualifications
Doctor of Physical Therapy (DPT) degree, or foreign equivalent when applicable. Required if license obtained on or after January 2016.
Current Physical Therapy license valid for practice in Oregon. Required to start the assignment.
Experience in home health settings. Required.
Experience with Home Care Home Base and PointClickCare (PointCare). Required.
Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification from an approved vendor per agency policy. Required.
Valid driver’s license and at least 21 years of age. Required.

Preferred Qualifications
Experience serving as a co-leader in interdisciplinary patient care teams. Preferred.
Prior hospital-based home health experience. Preferred.

Assignment Details
Shift Name: Days.
Workweek: 40 hours per week.
Home health fieldwork required; mileage included.
