Eligibility Requirements: To qualify for each quarterly payment, the employee must meet all of the following criteria during the applicable quarter: Maintain active employment status Have no disciplinary actions Have no tardiness occurrences Have no unexcused absences Failure to meet any of the above requirements during a given quarter will result in forfeiture of that quarter’s bonus payment.
