Registered Nurse Case Manager

Location: Fort Walton Beach, Florida
Job Type: Permanent
Shift: Days – 8:00 AM to 4:30 PM (Rotating Weekends)

Compensation

• Salary Range:
  • Minimum: $34.25/hr
  • Mid-Level (10 Years Experience): $42.82/hr
  • Maximum (15+ Years Experience): $47.10/hr

Sign-On Bonus

Total Bonus: $10,000

Paid over a 2-year commitment:

• $2,000 on 1st or 2nd paycheck
• $2,000 at 6 months
• $2,000 at 12 months
• $2,000 at 18 months
• $2,000 at 24 months

Position Summary

We are seeking a compassionate and experienced Registered Nurse Case Manager to support patients and families throughout their healthcare journey. This role focuses on coordinating patient care, developing individualized treatment plans, facilitating discharge planning, and ensuring safe transitions across the continuum of care.

The RN Case Manager collaborates closely with physicians, nurses, leadership teams, and interdisciplinary departments to deliver high-quality, patient-centered care.

Key ResponsibilitiesPatient & Family Support

• Guide patients and families through program orientation and care processes
• Educate patients regarding rehabilitation philosophy, insurance benefits, discharge criteria, and patient rights
• Coordinate family conferences and interdisciplinary treatment planning meetings
• Provide ongoing communication and emotional support throughout hospitalization

Care Coordination & Discharge Planning

• Develop individualized treatment and recovery plans
• Complete psychosocial assessments and discharge planning evaluations
• Identify barriers to discharge and coordinate appropriate resources
• Ensure smooth and safe patient transitions to appropriate levels of care
• Maintain accurate and timely documentation within the medical record

Collaboration & Leadership

• Collaborate with physicians, nursing staff, rehabilitation leadership, and interdisciplinary teams
• Promote patient advocacy and continuity of care
• Support organizational values focused on compassion, respect, and excellence

Required QualificationsEducation & Licensure

• Active Registered Nurse (RN) license
• Associate Degree or Certificate in Nursing required

Experience

• Minimum 2 years of recent acute care experience within the last 3 years
• Preferred backgrounds include:
  • Intensive Care Unit (ICU)
  • Coronary Care Unit (CCU)
  • Medical-Surgical
  • Long-Term Acute Care (LTAC)
  • Case Management
