Registered Nurse – Home Health

Schedule: 8-hour shifts

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40 hours per week

Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• 1-2 years of experience as a Registered Nurse in a clinical care setting
• 2+ years of Home health experience
• Proof of current CPR certification
• Current driver's license
• Must have reliable transportation and insurance

Roles & Responsibilities:

• Assess patients in home health and/or hospice settings to identify their physical, psychosocial, and environmental needs.
• Implement and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and families on care, medications, and safety, promoting self-care practices.
• Monitor and assess pain, utilizing appropriate management techniques.
• Provide clinical direction to support staff to ensure quality service delivery.
• Communicate effectively with patients, families, team members, and other healthcare professionals.

• Must maintain confidentiality.
• Ability to work flexible hours and travel locally.
