MDS Coordinator: Resident Assessments & Care Planning Lead

Stonebridge Senior Living

StoneBridge Senior Living is seeking a skilled MDS Coordinator to lead the RAI process across residents in a skilled nursing setting. The role focuses on accurate assessments, care plans, and collaboration with the interdisciplinary team to optimize residents' function and quality of life. Clinical experience is required, with RN preferred and RAC certification preferred. Proficiency in Point Click Care and strong communication are essential to coordinate care with residents, families, and staff. #J-18808-Ljbffr
