Key Responsibilities

• Provide physical therapy evaluations and treatments to residents in a skilled nursing facility setting
• Develop and implement individualized care plans to support patient rehabilitation and functional goals
• Collaborate with interdisciplinary team members to coordinate resident care
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards

Required Qualifications

• Active New York State Physical Therapist license required
• BLS certification required
• Prior experience working in a Skilled Nursing Facility required

Preferred Experience / Skills

• Experience with geriatric patient populations
• Familiarity with long-term care documentation and compliance requirements
