Responsibilities
- Assess patients, establish and implement individualized physical therapy treatment plans based on evaluation findings.
- Provide therapeutic interventions including therapeutic exercise, gait and balance training, range of motion, and manual therapy as appropriate.
- Monitor and document patient progress, adjust treatment plans, and communicate updates to the interdisciplinary team.
- Ensure patient safety, apply infection control protocols, and comply with regulatory and facility policies.
- Coordinate scheduling, discharge planning, and patient/ caregiver education, including home exercise programs as indicated.

Requirements
- Availability to start on 08/24/2026 and commit to a 13-week travel assignment.
- Ability to work 8-hour shifts, with 35 guaranteed hours per week.
- Willingness to work in a skilled nursing facility setting.
- Strong communication, organization, and teamwork skills.
- Ability to follow facility policies and documentation requirements; able to adapt to a fast-paced rehab environment.
