San Simeon by the Sound is seeking compassionate, skilled, and dedicated Licensed Practical Nurses (LPNs) to join our nursing team. The LPN is responsible for providing direct nursing care to residents, administering medications and treatments, monitoring resident conditions, and collaborating with the interdisciplinary team to ensure exceptional resident care and outcomes.

• Administer medications and treatments as prescribed by physicians.

• Monitor residents for changes in condition and promptly report findings to the RN Supervisor, Unit Manager, or physician as appropriate.

• Complete accurate and timely nursing documentation.

• Perform resident assessments within the scope of practice.

• Supervise and direct Certified Nursing Assistants (CNAs).

• Assist with admissions, discharges, and transfers.

• Maintain infection control and safety standards.

• Participate in care planning and interdisciplinary team meetings.

• Ensure compliance with facility policies, state regulations, and nursing standards of practice.

• Provide compassionate, resident-centered care while maintaining resident dignity and respect.

• Current New York State Licensed Practical Nurse (LPN) license in good standing.

• Long-term care, skilled nursing, or rehabilitation experience preferred.

• Strong clinical assessment and organizational skills.

• Excellent communication and interpersonal skills.

• Ability to work effectively in a fast-paced environment.

• Reliable attendance and punctuality.
