Responsibilities

• Provide direct nursing care to residents in a long-term care setting under facility policy and physician orders
• Administer prescribed medications and document accurately in the patient record
• Monitor residents' health status, assess vital signs, identify changes, and report to the supervising nurse
• Participate in care planning, collaborate with the interdisciplinary team, and maintain clear, timely documentation
• Support safe resident care through infection control practices, safety protocols, and quality standards
• Communicate effectively with residents, families, and the care team to ensure continuity of care

Requirements

• Active Licensed Practical Nurse (LPN) license; valid for practice in the United States
• Basic Life Support (BLS) certification
• Experience in long-term care or skilled nursing settings preferred
• Ability to work the evening shift (3:00 pm–11:30 pm) and meet guaranteed-hour expectations
• Strong clinical documentation and communication skills
• Education: graduate of an approved practical nursing program or equivalent credential
