Open to NPs (FNP, ANP, AGNP) and PAs, with licensure and post-graduate experience in geriatric medicine preferred.
Roles include collaborating with a multidisciplinary team, and working weekdays in a homecare setting with strong support from Hartford HealthCare leadership.
