Please view our benefits page Job Requirements: Master's or Doctoral Degree Licensed Independent Social Worker (LISW); OR Licensed Professional Clinical Consultant (LPCC); OR Doctor of Psychology (PsyD)/Doctor of Philosophy (PhD) Generalist knowledge of a wide variety of behavioral health conditions and evidence-based treatment modalities to serve diverse groups of patients across the lifespan Must be able to work in a team-based integrated brief treatment model Up to 1 year experience as a Professional Clinical Social Worker or Clinical Consultant Job Overview: The purpose of this position is to serve as part of the ambulatory treatment team providing direct behavioral health services to patients and families. Duties may include consultation, assessment, individual/family/group counseling/psychotherapy, education, prevention, medical exam room-based intervention, referral and service coordination with other community agencies and programs. Assist with program development and evaluation as directed. Perform other clinical, administrative, educational and/or research duties (commensurate with training and ethical scope of practice) as needed. Job Responsibilities: Clinical Service: Provide behavioral health services including assessment, evidence-based treatment, clinical interview administration, standardized assessment tools, diagnosis of BH conditions, psychotherapy/counseling, and supporting documentation. Familiar with evidence-based interventions such as motivational interviewing, problem focused therapy, mindfulness, CBT, and ACT. Assist patients in developing skills to facilitate disease self-management. Consultation and Collaboration: Develop and maintain collaborative relationships with the primary care provider, ambulatory practice team, behavioral health team members, patients and families. Provide behavioral health education to staff, patients, and families in individual and group settings. Work with community mental health providers and community support services. Case Management and Treatment Coordination: Work with the primary care team to provide referral, case management and follow up services. Engage patients, initiate referrals, address barriers to care and follow-up. Familiar with community resources for psychosocial and behavioral health needs. Documentation: Provide accurate and timely documentation in Epic, Agility or other authorized electronic health records. Assist in capturing program evaluation and fidelity measures as directed. Quality Improvement: Participate in meetings and quality improvement activities, continuing education, community education, and serve as a member of committees or related activities. Assist in monitoring the behavioral health program and identifying areas for improvement. Supervisory Support: May be asked to provide supervisory support to other team members, including professional guidance, assessment of supervisee's performance, approval of intervention and implementation plans, and assume responsibility for the welfare of supervisee's clients. Other Job-Related Information: Incumbent must be an independently licensed clinical social worker or Licensed Professional Clinical Consultant, experienced in providing consultation, assessment, and treatment of behavioral health problems for patients across the lifespan and willing to work in a fast-paced team-based practice.
