We're looking for a driven Long Term Care Physician – Post-Acute Care Doctor who's up for a challenge, ready to lead, and eager to make a lasting impact in a rapidly growing care model.
WHAT WE OFFER
Pay: $250,000+/year
Schedule: This position is full-time.

POSITION DETAILS:
Each day looks different, but expect to:
- Lead care teams in skilled nursing facilities
- Manage complex cases discharged early from hospitals
- Collaborate with Nurse Practitioners and specialists to deliver hospital-level care in a long-term setting
- Ensure continuity, quality, and compassion in every patient interaction
What You Need:
- Board certification or board-eligible internal medicine, geriatrics, critical care, or family practice
Previous experience as a hospitalist or in leadership positions is strongly preferred.
