Registered Nurse (RN) - Days

As a Registered Nurse Case Manager, you will provide direct care to patients in their homes or care facilities. Your role goes beyond offering medical assistance; it involves attentively listening to patients' needs and respecting their wishes. Building trusting relationships with patients and their families is a cornerstone of this position, allowing you to serve as the primary communication link within our interdisciplinary team of holistic care providers. An active, unencumbered state Registered Nurselicense. Current driver's license and auto liability coverage. Interest and Ability in providing patient care in homes and facilities.
