Responsibilities include clinical evaluation and treatment for acute, restorative or maintenance levels in Day Center, Nursing Home or in-home settings, integration of treatment plans into the Interdisciplinary Team plan of care, and assistance with ordering, inventory, distribution and maintenance of durable medical equipment. Schedule Monday-Friday 8:00 AM-5:00 PM, occasional weekends and evenings possible. Pay Rate $98,000 - $107,000 (Based on experience) Required Qualifications Graduate from a physical therapy curriculum approved by the American Physical Therapy Association and currently licensed as a Physical Therapist in Kentucky. Minimum of one‑year experience working with a frail or elderly population. Must clear the background check. Must have a valid driver's license and reliable transportation. Must have medical clearance for communicable diseases and up‑to‑date immunizations before having direct participant contact. Essentials Conduct initial comprehensive in‑person physical therapy assessment for all participants upon enrollment. Leads ongoing in‑person physical therapy re‑assessment minimally every six months or as needed. Participates in Interdisciplinary Team meetings, maintaining regular attendance and collaborating on plan of care decisions and 24‑hour care coordination. Provides treatment to participants, implementing procedures that meet identified goals, grading activities to facilitate progress, and consulting with the team on care issues. Evaluates participant needs, follows up during treatment, explains precautions, discusses condition and treatment plans, and recommends solutions to care problems. Assesses need for durable medical equipment, specialized shoes, and prostheses; obtains authorization prior to purchases and verifies fit and appropriateness of new orthotic items. Acts as liaison with nursing home and assisted living staff regarding physical therapy needs of resident participants.
