Key Responsibilities\n- \n
- Utilize your education, training, and clinical experience to provide accurate psychosocial diagnoses for patients facing end-of-life issues. \n
- Work closely with patients to create tailored, patient-centered treatment plans that prioritize their emotional and psychological needs. \n
- Implement behavioral interventions based on evidence-based practices, including techniques such as motivational interviewing, behavioral activation, and problem-solving therapy. \n
- Collaborate systematically with multidisciplinary teams, including physicians, psychologists, and other healthcare providers, to ensure holistic and integrated patient care. \n
- Regularly assess and monitor patient progress towards their therapeutic goals through virtual sessions, tracking treatment outcomes and making necessary adjustments. \n
- Support the management of psychotropic medications by coordinating with medical providers, monitoring adherence, side effects, and overall treatment efficacy. \n
In this role, you will focus on patients receiving hospice and palliative care. These individuals often face complex emotional challenges associated with terminal illnesses. Our approach is patient-centered, emphasizing dignity, respect, and empowerment, offering psychological support not only to patients but also to their families, which is essential in such sensitive settings.
Qualifications\n- \n
- A Master’s degree in Social Work from an accredited program is required. \n
- A current, valid, and unrestricted license to practice independently in mental health as a Licensed Clinical Social Worker (LCSW) in the state of New York. \n
- At least 1 year of experience working as an independently licensed therapist is necessary. \n

- Experience working in hospice or palliative care environments is preferred but not mandatory. \n
- Familiarity with telehealth practices and remote patient engagement strategies is advantageous. \n
- Knowledge of community resources related to end-of-life care will enhance your effectiveness in this role. \n
The starting salary for this position ranges from USD 80,000 to USD 100,000 annually, depending on qualifications and experience, with opportunities for performance-based bonuses. Additional benefits include:
