Company Description PACE Southeast Michigan is part of the Program of All-Inclusive Care for the Elderly, dedicated to helping older adults remain independent in their own homes for as long as possible. The organization uses an interdisciplinary team model that promotes close relationships with participants and their caregivers. Team members meet daily to proactively coordinate holistic care, focusing on prevention of avoidable emergency visits, hospitalizations, and nursing home placements. This approach supports higher quality, person-centered care tailored to the needs of each participant. PACE Southeast Michigan serves older adults in Wayne, Macomb, and Oakland Counties.

Role Description This full-time, on-site Medical Doctor role is based at PACE Southeast Michigan in Dearborn Heights, MI. The Medical Doctor will provide primary and geriatric care to PACE participants, including comprehensive assessments, diagnosis, treatment planning, and ongoing management of acute and chronic conditions. Daily responsibilities include collaborating with an interdisciplinary team (nursing, social work, therapy, pharmacy, and other disciplines) to coordinate individualized care plans, review clinical updates, and adjust interventions to support participants’ health and independence. The physician will conduct routine and urgent visits, monitor medication regimens, oversee preventive care and screenings, and ensure appropriate referrals to specialists and hospital services when needed. The role also involves documentation in electronic health records, participation in care conferences, and support for caregivers through education and clear communication about care plans and goals of care

Qualifications

• Current MD or DO degree from an accredited medical school and completion of an accredited residency program (Internal Medicine, Family Medicine, or Geriatrics preferred).
• Active, unrestricted Michigan medical license and DEA registration, or eligibility to obtain these prior to start date.
• Clinical expertise in primary care for adults and older adults, including management of complex chronic conditions, polypharmacy, and geriatric syndromes.
• Demonstrated ability to collaborate effectively within an interdisciplinary care team and to coordinate holistic, person-centered care.
• Strong skills in patient assessment, clinical decision-making, and evidence-based treatment planning.
• Proficiency with electronic health records, documentation standards, and regulatory/compliance requirements in healthcare.
• Excellent communication skills, including the ability to explain medical information clearly to participants, families, and caregivers.
• Commitment to non-discriminatory, culturally sensitive care and respect for the dignity and autonomy of older adults.
• Prior experience in PACE, long-term care, home-based care, or geriatric practice is highly beneficial.
• Ability to work on-site in Dearborn Heights, MI, and travel within the service area as needed for participant care.
