Education:

Bachelor's degree in Occupational Therapy

Licenses and Certification:

Illinois license in Occupational Therapy or eligible BLS

Experience and Skills:

Technical Experience: N/A

Performs thorough evaluation

Develops appropriate Plan of Care

Implements Plan of Care

Documentation is accurate, complete and timely

Provides supervision to assistants, rehab techs, and students

Compensation (Commensurate with experience):
