RN Case Manager - Care Coordination & Discharge

Huntsville Hospital is seeking an RN Case Manager to support physicians and an interdisciplinary team in coordinating patient care and improving clinical outcomes while managing care costs. The role integrates utilization management, care coordination and discharge planning.
