Case ManagerProvides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plansPerforms treatment(s) ordered by the physician and delegated by the R.N. Case ManagerObserves signs and symptoms and reports them to the R.N. Case ManagerNotes changes in physical or emotional status of clientsPromotes client and caregiver independence in self-careUtilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of dataFollows Agency policies regarding universal precautions, and safety guidelinesFulfills other assignments as requested by the R.N.
