Location: Jeffersonville, IN
Pay Rate: $56.14/hour

Schedule

• Monday – Friday
• 8:00 AM – 5:00 PM
• Rotating weekend on-call schedule required
• Onsite position

Job Summary

We are seeking a dedicated Registered Nurse Case Manager to provide comprehensive care coordination for high-risk patients. This role involves collaborating with healthcare providers, coordinating transitions of care, developing individualized care plans, and ensuring patients receive appropriate, cost-effective services while achieving the best possible health outcomes.

Responsibilities

• Perform comprehensive assessments of high-risk members to identify healthcare needs.
• Develop and implement individualized care plans in collaboration with primary care providers.
• Evaluate and reassess appropriate, cost-effective levels of care.
• Coordinate care transitions with physicians, social workers, and other healthcare professionals.
• Engage specialty resources and community services to support patient care.
• Maintain detailed documentation of clinical, functional, and financial outcomes.
• Advocate for patients while protecting their privacy and confidentiality.
• Monitor patient progress and adjust care plans as needed.
• Ensure compliance with clinical standards, regulatory requirements, and organizational policies.

Qualifications

• Associate's or Bachelor's Degree in Nursing or a related field.
• Current, unrestricted Registered Nurse (RN) license in the state of Indiana.
• Case Management certification required.
• Strong assessment, care coordination, and communication skills.
• Ability to collaborate effectively with interdisciplinary healthcare teams.
• Excellent documentation and organizational skills.

Additional Information

• Onsite position located in Jeffersonville, IN.
• Participation in an on-call rotation, including weekends, is required.
