Physical Therapist (Home Health)
Excellent Pay + Paid Mileage + Flexible CompensationFreedom at Home is a Veteran-Owned Home Health Agency serving Southeast Michigan. We're looking for a compassionate, motivated Physical Therapist to join our growing team.

Opportunity to make a real difference in patients' lives
Evaluate patients in their homes and develop individualized treatment plans.
Provide skilled physical therapy services.
Educate patients and caregivers on exercises, safety, and mobility.
Complete OASIS and clinical documentation in a timely manner.
Collaborate with physicians, nurses, and other healthcare professionals.
Monitor patient progress and adjust treatment plans as needed.
Promote safe, high-quality patient care and excellent outcomes.
Graduate of an accredited Physical Therapy program.
Current Michigan Physical Therapist license.
Current CPR certification.
Minimum of 2 years of PT experience preferred.
Home Health experience is a plus but not required.
Valid driver's license, reliable transportation, and auto insurance.
