Responsibilities: Develop and implement personalized care plans for clients, taking into account their medical, physical, and emotional needs Coordinate and supervise care services, including home health, hospice, and palliative care, to ensure seamless transitions and effective care delivery Collaborate with clients, family members, healthcare providers, and other stakeholders to ensure that care goals are met and client needs are met Conduct assessments, evaluations, and progress reports to ensure optimal client outcomes and quality of care Manage and develop staff, providing guidance and support to ensure effective care delivery and staff development Maintain accurate records and reports, adhering to regulatory requirements and company policies Requirements: 3-5 years of experience in care management, healthcare, or a related field Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple projects simultaneously Excellent communication, interpersonal, and organizational skills, with the ability to work effectively with diverse stakeholders Ability to work in a fast-paced environment, with flexibility to accommodate changing priorities and schedules Strong attention to detail, with the ability to maintain accurate records and reports #J-18808-Ljbffr
