Job Description

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans according to patient needs, goals, and physician direction. Responsibilities include ensuring the design of functional treatment programs, educating patients, families, and coworkers, and upholding the organization’s Mission Statement and Core Values.

Responsibilities

• Conduct comprehensive evaluations of patients and create individualized treatment plans focused on progress towards discharge.
• Document treatment in a clear, accurate, and timely manner, ensuring compliance with state and federal guidelines.
• Promote patient and employee safety by adhering to safety policies and identifying workplace hazards.
• Participate in student programs, providing a positive learning environment and appropriate supervision.
• Engage in ongoing professional development through education and participation in professional organizations.
• Maintain professional licensure in good standing and comply with continuing education requirements.
• Follow performance improvement initiatives to enhance the quality of services.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an Accredited School
• 6 months - 1+ year of Outpatient Clinical Experience
• Current CPR Certification
• Active/applicable license for the State where work will be performed
