Domiciliary Mental Health Registered Nurse
The Domiciliary Mental Health Registered Nurse is responsible for providing and leading the provision of competent, evidence-based care to assigned patients. The domiciliary RN will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation. The domiciliary RN provides patient-centered and recovery-oriented care and maintains self-awareness while caring for patients living with substance use disorder and/or MH conditions. Duties include but are not limited to:
- Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
- Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.
- Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs.
Veteran/Patient Driven Care:
- Improves Veteran/patient driven care by resolving experience issues such as coordination of care, timeliness or process issues, common themes of dissatisfaction, or environment of care concerns for the immediate practice setting.
- Applies patient, family, and caregiver knowledge, values, and beliefs into care delivery without judgement or discrimination.
- Serves as an advocate for patients, families, and caregivers, improving outcomes for the immediate practice setting.
- Serves as a resource for patients, caregivers, and staff to address and resolve ethical issues in the immediate practice setting.
- Leads the implementation of high reliability principles and I CARE values to increase consistency of care delivered and improve patient outcomes.
Evidence-Based Practice/Research:
- Applies evidence-based practice/research resulting in improved outcomes in the immediate practice setting.
- Assesses the immediate practice setting to identify efficient, cost-effective care delivery.
- Leads patient care improvement through monitoring, analyzing, and evaluating care outcomes in the immediate practice setting.
- Leads initiatives for expanding best techniques to identify and apply patient preferences to shared care delivery decisions in the immediate practice settings.
- Performs additional duties as assigned.
