Registered Nurse (RN)

Position Overview
We are seeking a compassionate, experienced, and clinically skilled Registered Nurse (RN)to join our healthcare team on a full-time basis.The RN provides comprehensive nursing care and clinical oversight to residents in a skilled nursing and post-acute setting. This role requires strong clinical judgment, assessment skills, communication, and leadership to ensure residents receive safe, timely, and high-quality care.
Key Responsibilities

• Perform comprehensive nursing assessments and evaluate residents' health status.
• Identify and respond appropriately to changes in residents' conditions.
• Develop, initiate, implement, and update nursing care plans in collaboration with the healthcare team.
• Provide direct nursing care and oversee the delivery of resident care.
• Administer medications and treatments in accordance with physician orders, facility policies, and applicable regulations.
• Monitor residents' responses to medications, treatments, and interventions.
• Make appropriate clinical decisions and escalate significant changes in condition to physicians and other healthcare providers.
• Delegate and supervise appropriate nursing and resident-care duties performed by LVNs/LPNs, CNAs, and other care team members.
• Communicate effectively with physicians, residents, families, and members of the interdisciplinary healthcare team.
• Maintain accurate, timely, and complete clinical documentation.
• Participate in care conferences and collaborate with the interdisciplinary team to support resident goals and outcomes.
• Provide guidance, support, and direction to nursing and care team members.
• Follow infection prevention, medication safety, emergency response, and facility protocols.
• Promote resident dignity, privacy, safety, and quality of life at all times.

Qualifications

• Current and active RN license in the state of practice.
• Graduate of an accredited nursing program.
• Current CPR/BLS certification.
• Strong clinical assessment, care planning, documentation, and delegation skills.
• Strong communication, organization, leadership, and critical-thinking abilities.
• Ability to manage multiple resident-care priorities in a fast-paced healthcare environment.
• Previous SNF, acute care, or long-term care experience preferred.

Scope of the RN Role
The RN provides higher-level clinical oversight and nursing judgment within the skilled nursing and post-acute setting. Responsibilities include comprehensive nursing assessments, evaluation of significant changes in condition, development and updating of nursing care plans, clinical decision-making, delegation and supervision of appropriate duties, and coordination of care with physicians, residents, families, and the interdisciplinary team.The RN serves as an important clinical resource for the nursing team and works collaboratively with LVNs/LPNs, CNAs, physicians, and other healthcare professionals to ensure residents receive safe, appropriate, and individualized care.
