Responsibilities
- Conduct initial evaluations and ongoing assessments of residents to determine physical therapy needs and goals.
- Develop, implement, and adjust individualized treatment plans addressing mobility, gait, strength, balance, endurance, and functional outcomes.
- Provide direct patient care, supervise therapy sessions, and modify interventions based on progress and safety considerations.
- Document evaluation results, plan of care, progress notes, outcomes measures, and discharge summaries in a timely, accurate manner.
- Collaborate with nursing staff, therapists assistants, and interdisciplinary team members to coordinate care and optimize residents’ outcomes.
- Ensure patient and staff safety, adhere to infection prevention standards, and utilize equipment and modalities appropriately.

Requirements
- Active Physical Therapist license in North Carolina
- Eligibility to work in the United States and participate in a travel assignment
- Ability to work the stated schedule (08:00-16:30) with guaranteed hours
- Experience in a skilled nursing facility or geriatric rehabilitation setting is beneficial
- Strong communication, documentation, and teamwork skills; comfortable coordinating with multiple disciplines
