Clinical Support Partner (EMT) Garden Grove, CA (Weekend Shift)

At Healthcare in Action, we serve patients experiencing homelessness through an innovative “street medicine” approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems.

Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.

The Garden Grove Mobile Crisis Response Team (MCRT) is a co-response program operated by Healthcare in Action (HIA) in partnership with the City of Garden Grove and the Garden Grove Police Department (GGPD), delivering mobile mental health and crisis stabilization services in the field. The program provides eight hours of coverage per day, seven days per week, through two field teams, each pairing an EMT with an associate-level behavioral health clinician—an Associate Clinical Social Worker (ACSW), Associate Marriage and Family Therapist (AMFT), or Associate Professional Clinical Counselor (APCC)—who has prior crisis or field-based behavioral health experience.

The EMT / Clinical Support Partner serves as the medical and operational partner on a two-person MCRT field team, working alongside an associate-level behavioral health clinician to respond safely to crisis calls with GGPD. The EMT operates the response vehicle, maintains communication with GGPD, performs basic medical assessment and monitoring, supports de-escalation and scene safety, and helps connect individuals to appropriate medical, behavioral health, and social services under the co-response model.

Responsibilities:

• Operate and maintain the response vehicle, ensuring the team arrives safely and that the vehicle is stocked, clean, and ready for each shift.
• Maintain communication with GGPD regarding team location, status, and plan, and respond only after GGPD clears the scene.
• Perform basic medical assessment and monitoring, and determine when a higher level of medical care or EMS is needed.
• Support de-escalation, triage, and assessment alongside the clinician partner, providing a calm, trauma-informed presence that keeps the individual, the team, and bystanders safe.
• Support scene logistics and other appropriate tasks, including gathering information, coordinating with first responders, and arranging transportation as needed.
• Assist in connecting individuals to appropriate services, including HIA’s street medicine team, a community provider, the CAT team, or the Department of Mental Health (DMH).
• Operate within the approved scope of work by observing program boundaries (no involuntary holds, no transport of individuals in program vehicles) and escalating to GGPD if a safety concern arises.
• Assist with inventory of program supplies and equipment, keeping medical and safety supplies appropriately stocked and following company policies for the handling of biohazardous waste.
• Complete timely, accurate documentation of each response in the applicable systems and support program reporting requirements.
• Protect confidential information by following all HIPAA regulations; actively support the achievement of SCAN’s and HIA’s Vision and Goals; and perform other duties as assigned. This position is a patient-facing role and a healthcare worker.

Qualifications:

• High School Diploma or GED
• Current, unrestricted California EMT certification required.
• Current BLS/CPR certification required.
• Valid California Driver’s License required.
• 2 or more years of experience as an EMT, or in a comparable field, EMS, or hospital setting.
• Experience in crisis response, street medicine, or field-based work with vulnerable populations preferred.
• Valid California Driver’s License and a clean driving record sufficient to operate a program vehicle.
• Experience working alongside law enforcement or in a co-response model, and work with individuals experiencing homelessness, are a plus.
• Work is primarily field-based in Garden Grove; some onsite/administrative time as needed.
• Strong de-escalation skills and the ability to remain calm and resourceful in unpredictable, high-stress field situations.
• Excellent assessment skills; ability to problem-solve and prioritize changing conditions in the field as part of a two-person team.
• Knowledge of escalation pathways (CAT team, Police, DMH) and ability to maintain quality, safety, and infection-control standards.
• Strong interpersonal, written, and verbal communication skills; strong organizational, decision-making, and multitasking skills.
• Ability to appropriately maintain confidentiality and protect PHI.
• Ability to be culturally sensitive and work with a diverse, vulnerable population; knowledge of community and county-based resources for behavioral health and homelessness.
