Position Overview

We are seeking Licensed Social Workers (LSW) and Licensed Independent Social Workers (LISW) to join our team as Therapist/Behavioral Health Care Managers . Candidates must have their Masters Degree and be licensed as a Licensed Social Worker, Licensed Clinical Social Worker, Licensed Mental Health Counselor In this role, you will provide psychotherapy, group therapy, and collaborative care management to residents in need.

Key Responsibilities

• Deliver individual and group psychotherapy sessions tailored to the needs of residents.
• Facilitate collaborative care management by coordinating with psychiatrists, primary care providers, and other team members.
• Address the mental health needs of residents with chronic severe mental illness, substance use disorders, depression, and major neurocognitive disorders.
• Develop and implement therapeutic interventions using evidence-based modalities.
• Maintain detailed and accurate clinical documentation in an electronic medical records (EMR) system.
• Conducts initial behavioral health evaluations of residents as needed

Qualifications

• Current LSW, LCSW, LMHC licensure in Indiana.
• Master’s Degree in Social Work or Masters Degree in Counseling from an accredited institution required.
• Experience working with diverse patient populations, including those with chronic severe mental illness and major neurocognitive disorders.
• Proficiency in using electronic medical records (EMR) systems.
• Familiarity with evidence-based therapeutic modalities (preferred).
• Strong interpersonal and communication skills.
• Ability to work collaboratively within a multidisciplinary team.
