How You'll Contribute Implement treatment plan developed by supervising therapist using appropriate modalities. Seek consultation as necessary. Monitor patient's responses to the treatment plan. Regularly communicate patient progress and possible goal revisions to the physical therapist. Document services provided and patient response/progress. Educate the patient and family/caregiver about patient deficits. Assist with patient discharge planning. Provide information regarding appropriate selection/use of adaptive equipment and community support programs. What We're Looking For Education: Associate's degree required; graduate of a program in discipline required. Certifications: Basic Life Support (BLS).
