Support Coordinator – Case Manager

We are looking for a compassionate and motivated Support Coordinator – Case Manager who wants to make a meaningful difference in the lives of individuals with disabilities and their families.
As a Support Coordinator, you will work directly with individuals and families to understand their needs, identify goals, connect them with services and community resources, and advocate for the supports necessary to help them achieve greater independence and an improved quality of life. You will also coordinate services among participants, families, providers, and other members of the support team to ensure each person receives appropriate and person-centered care.
The ideal candidate will have experience in human services, case management, social services, or support coordination, along with strong communication, organization, documentation, and care-planning skills.
