Job Duties
The primary job outcomes for the Designated Coordinator will be to meet licensure standards for New Dawn Waivered Services and assist persons living in licensed programs in growing and developing to their fullest potential.
Activities to meet standards include: programming for people served, staffing, provision of medical care, assistance in house account and individual financial control, house maintenance and related areas. This employee provides supervision and assistance to person served at the required level in the home and community.
All activities regarding program development and implementation must be done with guidance from the Support Plan, (SP) and Support Plan Addendum, (SP addendum).
The Designated Coordinator will also provide direction and supervision to the Direct Service Professionals. The Designated Coordinator must work closely with professionals to develop program plans and guide implementation of programs.
All decisions regarding program development and implementation must be accompanied by the person served team input and are made within the framework of person centered planning and service delivery.
II. POSITION QUALIFICATIONS REQUIRED
A. Education
A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
A minimum of 50 hours of education and training related to human services and disabilities; and
Four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in 1 to 3 above.
B. Training
Must be currently CPR certified with recertification, when required.
Must be trained in First Aid and medication administration as part of orientation.
Successfully complete required orientation training within prescribed time lines.
Meet ongoing training requirements as specified by licensing rules and New Dawn present or future policy.
C. Personal Abilities
Must demonstrate independent ability in decision making skills particularly regarding programmatic issues.
Must be able to read, speak and understand English language well enough to communicate with persons served and co-workers, understand New Dawn policies and make entries on reports and records.
May not be disqualified from employment by Applicant Background Study.
Must comply with New Dawn, Inc.’s substance abuse policy.
D. Medical Requirements
Ability to lift a minimum of 50 pounds from floor to waist, demonstrating acceptable body mechanics. This includes but is not limited to the ability to carry groceries, aid with control of behavior if needed and assist with physical transfer and/or ambulation.
Assist co-workers in correctly and safely using equipment to assist persons served in standing, ambulating or having access to bathroom when needed.
