As a Hospice Admissions Nurse, you'll be the face of our care—helping families say "yes" to compassionate end-of-life support when it matters most.What You'll Do as a Hospice RN Admissions Nurse:Report directly to the Admissions Manager and serve as a key ambassador for our hospice servicesVisit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice careExplain hospice philosophy and services clearly, compassionately, and with clinical confidenceGuide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to careCollaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of careUse the nursing process to assess patient needs, support care planning, and document clinical findingsBuild trust and provide emotional reassurance to patients and families as they make this important decisionAccurately complete required documentation and ensure compliance with state and federal regulationsWork closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoffCompensation may vary within the salary range provided based on several factors including but not limited to a candidate's location, experience, education, skills, licensure, certifications and department equity.
