Role Overview Provides clinical management and diabetes self-management education to patients, collaborating with healthcare providers to develop individualized care plans. Provides clinical management and diabetes self-management education to patients, collaborating with healthcare providers to develop individualized care plans. Plans and delivers educational programs tailored to patients, families, and community needs, incorporating cultural considerations. Utilizes diverse teaching strategies to enhance patient understanding and engagement in diabetes care. Assists with nutritional assessments alongside registered dietitians and formulates personalized meal plans. Ensures compliance with regulatory standards and maintains accurate records for certification purposes. Participates in program development, promotes diabetes services, and supports a multidisciplinary approach to improve patient outcomes. Performs various nursing functions and collaborates with team members as needed. Qualifications & Skills Graduate of an accredited nursing program; must obtain Certified Diabetes Educator (CDE) certification within one year. Minimum of one year nursing experience in outpatient or acute care settings. Current RN licensure in NJ and BLS certification. Excellent organizational, communication, and interpersonal skills; strong knowledge of endocrine/diabetic care. Basic Computer Skills Required.
