PACE Social Worker

Responsibilities Coordinate all transitions of care, mental and psychosocial care. Counsel participants on financial and advanced care planning options including advance directives, DNR, etc. Advocate for participants and ensure participants’ wishes are enacted. Qualifications Master’s degree in Social Work (MSW) from an accredited institution is required. At least one year of experience working with the frail or elderly population is highly desirable. Current valid Driver’s License, proof of auto liability coverage, and reliable transportation required. Current LCSW certification is preferred. #J-18808-Ljbffr
