Home Health Licensed Practical Nurse

As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love. $5,000 sign on bonus . Responsibilities Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN. Implement individualized care plans in collaboration with the patient, family, and healthcare team. Educate patients and their families on disease management, treatment options, and self‑care techniques. Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes. Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes. Participate in care coordination activities and discharge planning. Use your skills to make an impact. Required Qualifications Current nursing license in the practicing state. Valid driver’s license, auto insurance and reliable transportation. Valid CPR certification. Two years experience as an LPN/LVN in a clinical setting. Preferred Qualifications Nursing experience in a Home Health or Hospice setting. Pay & Hours Pay per visit/unit: $33.00 – $47.00. Base annual salary: $53,000 – $73,000. Scheduled weekly hours: 40. Life insurance. Additional Information Tuberculosis (TB) Statement: This role is considered patient facing and is part of Humana's TB screening program. You will be required to be screened for TB. You must have a valid state driver’s license and maintain personal vehicle liability insurance in accordance with your residing state’s minimum required limits or higher protections.
