Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on the creation of a caring environment that promotes healing and wellness. This includes conducting assessments, treatments, and patient education. The RN will also facilitate admission, transfer, and discharge activities. Health information is obtained from families or other necessary sources. Both objective and subjective data are collected and documented. The RN verifies initial assessments when obtained by other healthcare team members. A problem list is developed, prioritizing patient needs with input from the inter-professional team. Actions and interventions are designed relevant to the patient's culture and age. Desired outcomes are determined in coordination with the patient and family. The RN implements patient care therapies and procedures with empowerment and clinical autonomy. Procedures, treatments, and processes are explained to patients and families. The RN takes action to alleviate patient stress and discomfort. Changes in patient condition are recognized, responded to, and communicated appropriately. Ongoing assessments and responses to procedures and therapies are conducted and documented. The RN sets priorities for the care of multiple patients. Resources are assured, and the plan of care is coordinated. Workload is organized for effective utilization of time and energy. Tasks are delegated appropriately while overseeing team members' performance. The accuracy and completeness of delegated patient care are monitored. Educational needs of patients are identified and addressed based on comprehension level. The discharge process is facilitated by executing discharge orders promptly.
