Compassionate Hospice RN Case Manager
Advocate Health
Advocate Health is seeking a Hospice Case Manager to identify and coordinate care for terminally ill patients and families in hospice settings. You will educate, assess, and support through routine contacts and care planning, leading the IDG to ensure quality end-of-life care. The role requires RN licensure, BLS, and a valid driver's license, with 1–2 years in home health, hospice, or related fields.
