ABOUT THE ROLE: To continue advancing our impact across the communities we serve, Commonpoint is seeking a Case Manager to join the A dult Workforce Vocational Training Department. The Adult Workforce Department plays a vital role in providing employment and vocational training services to individuals working toward long-term economic stability and career success. The Case Manager will provide comprehensive case management and employment support to vocational training and general workforce clients, helping them navigate barriers, access benefits and resources, successfully complete their training programs, graduate, and ultimately secure and retain competitive employment in their chosen field. The Case Manager will build strong relationships with clients and provide individualized guidance throughout their workforce journey, while carrying out additional responsibilities aligned with their case management, vocational, and employment goals. Schedule: This position is based out of Commonpoint Queens locations with occasional meetings in Commonpoint's Bronx Hub. The typical schedule may include Monday through Thursday, from 5:00 p.m. to 8:00 p.m., weekends, or a combination of weekday evenings and weekends, based on program and client needs. Flexibility is essential, as occasional weekend availability may be required to support case management and vocational training activities. ROLE RESPONSIBILITIES: Case Management: Provide comprehensive case management to Commonpoint's Adult Workforce Vocational training program participants from enrollment through job readiness, employment, and post-employment retention. Trainings include but are not limited to Construction, Allied Health, Digital Court Reporter, and other existing trainings. Complete comprehensive vocational assessment, and then vocational counseling and referrals to corresponding hard skills training. Deliver job readiness and employment services, including resume development, interview preparation, job search strategy, professional networking guidance, and direct employer referrals. Support participants in overcoming employment barriers by coordinating referrals to internal and external social services in collaboration with case management teams. Provide ongoing follow-up and retention services to participants after placement, implementing strategies that promote long-term employment success and address barriers to job retention. Track retention milestones (e.g., 30-, 60-, 90-, 180-, and 360-day) and employment outcomes, ensuring timely and accurate documentation in program databases and reporting systems. Collect, verify, and maintain accurate participant and employment documentation, including case notes, placement verifications, pay stubs, retention follow-ups, and other required records; ensure timely and accurate data entry in required reporting systems to support program compliance, performance tracking, and funder reporting requirements. Implement and administer participant surveys to evaluate program effectiveness, participant satisfaction, internship & employment outcomes, using findings to support continuous program improvement. Collaborate with program staff to meet monthly and quarterly performance goals and contribute to program improvement, reporting, and funding proposals as needed. Maintain strong digital proficiency, including Microsoft Office Suite and Google Workspace applications, to support data tracking, communication, and compliance reporting. People & Communications: Collaborate closely with Commonpoint leadership, program staff, and Case Managers across the Bronx and Queens to ensure seamless coordination of services, referrals, and educational support. Maintain consistent cross-site communication to align on participant progress, enrollment targets, internship placements, and employment outcomes. Facilitate and/or coordinate virtual workshops across both boroughs focused on professional development, credential readiness, financial literacy, and career advancement topics. Provide individualized coaching and direct support to adults with limited work history or educational attainment, ensuring equitable service delivery across locations. Identify and address employment and education barriers by connecting participants to relevant internal departments and external community partners. Partner cross-departmentally to develop and implement innovative engagement and placement strategies that drive measurable education and employment outcomes. Attend and actively participate in cross-borough staff meetings, training, and professional development sessions to promote best practice sharing and program alignment. Proactively communicate participant achievements, challenges, and retention updates with appropriate internal stakeholders to ensure coordinated and high-quality service delivery across sites. Data & Grant Reporting: Complete required reporting, including progress notes, client tracking, and timely data entry in accordance with program deadlines in required online CRM databases. Utilize data and performance metrics to monitor outcomes, track participant progress, and inform service delivery strategies. Assist with the preparation of program reports and support funding proposal development as needed. Assist with grant reports, funding proposals, and outcome documentation. QUALIFICATIONS & KEY ATTRIBUTES: MINIMUM QUALIFICATIONS: Associates or Bachelor’s Degree in Social Work, Human Services, Education, Business, Communications, Psychology, or related field highly preferred. At least one to two years relevant experience in workforce development, recruiting, human resources preferred. Sensitivity towards, and ability to, work with diverse individuals including, but not limited to, immigrants, young adults, single parents, English language learners and other low-income, high-barrier to employment populations. PREFERRED QUALIFICATIONS: Previous experience in a Commonpoint role is a plus. Bilingual skills are welcome. Demonstrated experience working in a goal-driven environment to achieve monthly targets. Demonstrated experience effectively teaching or facilitating groups and individuals to achieve a desired outcome using up-to-date techniques/methods. Experience providing comprehensive assessment and case management services and/or helping others to large caseload desirable. Excellent verbal and written communication and strong interpersonal skills required, ability to effectively conduct meetings, training, groups preferred. Experience with Salesforce, DYCD Connect, or other CRM databases preferred. Demonstrated experience working in a goal-driven environment to achieve monthly targets. KEY ATTRIBUTES FOR COORDINATOR ROLE: Organized and detail-oriented with strong multitasking abilities. Excellent customer service and communication skills. Sensitivity towards and ability to work with diverse individuals including, but not limited to, immigrants, young adults, single parents, English language learners, and other low-income, high-barrier-to-employment populations. Demonstrated experience working in a goal-driven environment to achieve monthly targets. Demonstrated experience teaching or facilitating groups and individuals to achieve a desired outcome using up-to-date techniques/methods. Experience providing comprehensive assessment and case management services and/or helping others with a large caseload is desirable. Excellent verbal and written communication and strong interpersonal skills are required. The ability to effectively conduct meetings, training, and groups is preferred.
