Employment Type: Part-Time About this position PHYSICAL THERAPIST (PT) – PARKVIEW HEALTH & REHABILITATION CENTER Work with a multidisciplinary team to ensure a high-quality rehabilitation experience for patients and their families by creating responsive and individualized treatment plans. Provide in-house therapy with local management support in a supportive rehab environment with progressive treatment modalities addressing a variety of diseases and impairments. Responsibilities Collaborate with the rehab team to develop and implement patient-centered treatment plans. Deliver physical therapy services to patients in accordance with state licensure and facility policies. Monitor and document patient progress, adjusting treatment plans as needed. Educate patients and families about treatment goals, procedures, and home exercise programs. Maintain clinical records and ensure compliance with regulatory and safety standards. Qualifications EDUCATION: Must be a graduate of an accredited therapy program. CERTIFICATE / LICENSE: Must be licensed in the state where the SNF is located.
