Summary of Position

Under the supervision of the Director of Clinical Services and County Nurse Managers, the Hybrid Clinical Assistant helps to facilitate patient care in the clinic environment. They are an essential part of the care team from the onset of the visit until its completion. They are one of the primary guides for the patient through the entire patient visits cycle. Tasks assigned to this role are both clinical and clerical. The clerical clinical assistant is responsible for documenting clinical and demographic information and accurately documenting this material in the electronic health record (EHR). Assisting with scheduling and monitoring hospital and ER follow ups. The Hybrid Clinical Medical Assistant are responsible for effective communication with patients, providers, and staff in multiple settings.

Minimum Qualifications

Interacts in a professional and patient-centered manner with patients. Able to work cooperatively with internal staff to deliver safe, effective, quality care to all patients. Able to plan, prioritze and complete assigned tasks with a high level of accuracy. Ability to work well under time constraints while maintaining accurate records.

· Experience: 1 year of related work preferred but not required; health care experience preferred.

· Additional skills required : Knowledge of medical office protocols/procedures. Knowledge of medical terminology. Familiarity with basic computer operations including Word, Excel, and Power Point. Proficiency in Spanish language preferred.

· Travel r equired - Ability to travel to all APHS locations upon request is required

Education : High school diploma or GED

Certification(s)/Licensure: Current Medical Assistant Certification required: CPR or BLS preferred.

· Frequently lifts, pushes or otherwise moves and positions patients or other objects, exerting up to 50 lbs.

· Repetitive movement of hands and fingers – typing and/or writing.

· Ability to communicate effectively.

· Possible exposure to hazardous fumes, airborne particles, toxic/caustic chemicals, bodily fluids, etc.

 

 

 

 

 

Key Responsibilities

 

1. Maintain accurate documentation in the patient’s EHR based on established documentation procedures.

2. Fulfill clerical responsibilities as assigned including but not limited to the following: obtaining lab/X-ray reports, hospital notes, and referral information. Complete forms/requisitions as needed; verify patient demographics. Also, must monitor and assist in management of provider’s clinical inbox, which includes medication refills, patient cases, notification of critical labs, categorizing and labelling clinical documents appropriately, scanning/uploading clinical documents to Athena EHR, and assisting in prior authorizations for medications and urgent/STAT imaging.

3. Ability to call patients with both abnormal and normal laboratory results after the provider interprets the lab results.

4. Assist Patients with completing paperwork and SDOH coordination during the check in process in conjunction with the quality department and operations at clinical sites determined by the DOCS

5. Ability to process, schedule, and document STAT referrals for medical providers and notify patients of time, date, and location of the referral appointment within a timely manner.

6. Schedules and Tracks patient via EMR and other electronic systems for inpatient hospitalization and Emergency Department discharge within specified time period

7. Approx 50 percent working with provider and 50 percent in clerical/administrative role

8. Works with operations to assist in patient scheduling with the call center by answering high volume calls.

9. Maintain patient confidentiality and protect operations by keeping protected health information confidential and secure.

10. Assist the DOCS and ACMO with logging clerical information for meetings and disseminating information as directed to ensure appropriate clinical work flow

11. Assist Medical Providers with communicating with patients regarding medications, symptoms, referrals, appointments, and paperwork.

12. Ensure culturally and linguistically appropriate services.

13. Assist in telephone follow-up with patients as directed by the provider or DOCS. 

14. Under the direction of the provider and/or DOCS, assists with patient health education and follow-up on established patient plan(s) of care

15. Complete in a timely manner, with adequate supervision, the assigned clinic maintenance tasks including but not limited to the following: setting up instruments and equipment, running tests and controls on equipment, cleaning exam/procedure rooms and documenting completed tasks based on clinic protocol.

16. Fulfill clerical responsibilities as assigned with adequate supervision including but not limited to the following: obtaining lab/X-ray reports, hospital notes and referral information. Complete forms/requisitions as needed; verify insurance coverage and patient demographics.

17. With supervision, assist provider with various procedures and accurately document results of completed procedures.

18. With adequate supervision, keep supplies ready by inventorying stock, placing orders, and verifying receipt of supplies.

19. Participate in mandatory in-services and drills, attend staff meetings and other trainings, including appropriate age specific educational trainings offered on regular basis for professional development. 

20. Participate in quality improvement activities by initiating or contributing to monitoring, measuring, analyzing, improving and/or controlling program goals, objectives and/or services.
