Registered Dietitian (RD/RDN) – Long-Term Care Consultant

Clinical Nutrition Consulting Services, LLC (CNCS) is seeking Registered Dietitians (RD/RDN) to join our growing clinical nutrition consulting team.

CNCS provides clinical nutrition and regulatory consulting services to long-term care, skilled nursing, post-acute, rehabilitation, and affiliated healthcare organizations. We are seeking Dietitians who are clinically strong, organized, dependable, and comfortable working independently while collaborating with facility leadership and interdisciplinary healthcare teams.

Opportunities are available for full-time, part-time, and PRN/contract positions , with assignments ranging from one facility to multiple sites based on employment status, geographic location, availability, and contracted service hours.

What You’ll Do

As a CNCS Registered Dietitian, you will manage the clinical nutrition needs of residents within your assigned facility or facilities while supporting regulatory compliance, quality improvement, and survey readiness.

Primary responsibilities include:

• Complete comprehensive nutrition assessments, reassessments, and timely clinical documentation.
• Identify, evaluate, and monitor residents at nutritional risk, including significant weight changes, malnutrition risk, wounds, dialysis, enteral nutrition, poor intake, altered diets, and other high-risk conditions.
• Develop individualized nutrition interventions and measurable resident-centered care plans.
• Monitor weight trends, food and fluid intake, supplement utilization, laboratory data, wound healing, and resident response to nutrition interventions.
• Lead or actively participate in Nutrition at Risk (NAR) meetings and other interdisciplinary clinical meetings.
• Collaborate with nursing, physicians/providers, MDS, speech therapy, rehabilitation, dietary services, and facility leadership.
• Complete nutrition-related MDS responsibilities, care plans, progress notes, follow-up assessments, and other required clinical documentation.
• Provide recommendations for therapeutic diets, oral nutrition supplements, fortified foods, enteral nutrition, texture modifications, and other appropriate nutrition interventions.
• Provide nutrition education and counseling to residents, families, staff, and interdisciplinary team members.
• Support dietary operations through clinical oversight, audits, menu review, tray accuracy, sanitation observations, and other contracted services.
• Participate in QAPI, quality assurance activities, survey preparation, Plans of Correction, and corrective action initiatives when applicable.
• Maintain clinical practices and documentation consistent with CMS requirements, applicable state regulations, professional standards, facility policies, and CNCS expectations.
• Complete assigned CNCS reports, audits, meetings, and contractual service requirements.
• Manage assigned contract hours effectively and proactively communicate clinical concerns, regulatory risks, and facility needs.

What a Typical Day May Look Like

• Your day may begin by reviewing new admissions, significant weight changes, residents at nutritional risk, physician orders, laboratory results, wounds, intake concerns, and residents requiring clinical follow-up.

• You may then complete resident assessments and interviews, review medical records and weight trends, evaluate food and fluid intake, develop or modify nutrition interventions, update care plans, and document recommendations in the facility’s electronic health record.

• Throughout the day, you will collaborate with nursing, dietary services, MDS, therapy, providers, and facility leadership. Depending on the facility schedule, you may also lead or participate in a Nutrition at Risk meeting, care plan meeting, QAPI review, clinical risk meeting, or survey-readiness activity.

• Before completing your scheduled services, you will ensure priority clinical concerns have been addressed, required documentation is current, appropriate follow-up has been established, and significant concerns have been communicated to the appropriate facility and CNCS leadership.

Required Qualifications

• Current Registered Dietitian Nutritionist (RDN/RD) credential through the Commission on Dietetic Registration (CDR).
• Current dietitian license in the applicable state(s) of practice where licensure is required.
• Bachelor's degree or higher meeting applicable professional credentialing requirements.
• Strong clinical assessment and documentation skills.
• Ability to work independently and manage assigned caseloads and contract hours.
• Strong organizational, time-management, and professional communication skills.
• Ability to collaborate effectively with interdisciplinary healthcare teams.
• Proficiency with electronic documentation/EHR systems and standard business technology.
• Ability and willingness to travel to assigned facilities as required.

Preferred Experience

Long-term care or skilled nursing experience is preferred, but we will consider strong clinical candidates who are interested in developing expertise in long-term care.

Experience in any of the following is especially valuable:

• Long-term care, skilled nursing, post-acute, or rehabilitation
• Nutrition Care Process (NCP)
• MDS and interdisciplinary care planning
• Significant weight-change management
• Malnutrition identification and intervention
• Enteral nutrition
• Wound-related nutrition management
• Nutrition at Risk meetings
• CMS survey preparation and regulatory compliance
• QAPI and quality improvement
• PointClickCare or similar EHR systems

Who You’ll Report To

CNCS Registered Dietitians function as clinical consultants within their assigned facilities and are accountable to Clinical Nutrition Consulting Services, LLC leadership .

The position ultimately reports to the Chief Executive Officer (CEO) and may receive clinical, quality assurance, operational, and account support from designated CNCS leadership.

Within each assigned facility, the Dietitian works collaboratively with the Administrator, Director of Nursing, Food and Nutrition Services leadership, MDS team, providers, therapy departments, and other members of the interdisciplinary team.

Single-Site & Multi-Site Opportunities

CNCS offers assignments based on facility needs, geographic location, contracted hours, and employment status.

Opportunities may include:

Single-Site Consulting: Primary responsibility for one facility based on contracted clinical nutrition hours.

Multi-Site Consulting: Responsibility for multiple facilities within a geographic area, with scheduled service hours assigned to each account.

PRN/Coverage Consulting: Coverage for PTO, leaves of absence, vacancies, temporary staffing needs, surveys, or other short-term clinical needs.
