Palliative Care Nurse Practitioner

We’re looking for a Nurse Practitioner with a passion for palliative and/or hospice care to join our growing team. This is an opportunity to make a meaningful impact by supporting patients and families through complex health journeys with dignity and compassion. What You’ll Do Provide comprehensive, patient-centered care to older adults in palliative or hospice settings Conduct assessments, diagnose conditions, and develop individualized care plans Collaborate with physicians and interdisciplinary teams to improve patient outcomesSupport patients and families through serious illness, focusing on comfort and quality of life Prescribe and manage treatments in accordance with state guidelines and collaborative agreements Educate patients, caregivers, and staff on care plans and disease progression Coordinate care across settings and connect patients to appropriate resources What We’re Looking For Certification as an Adult or Geriatric Nurse Practitioner Experience in palliative care, hospice, geriatrics, or complex care management preferred Palliative Care certification (or willingness to obtain) Strong clinical judgment and ability to work independently Passion for working with older adults and supporting families #J-18808-Ljbffr
