Position Purpose: The Care Team Lead plays a key role in maintaining a high standard of care, ensuring caregiver and client satisfaction, and supporting overall operations of client / caregiver relationships.

Position Summary: The primary job of the Care Team Lead combines direct care duties with leadership responsibilities, including quality assurance, caregiver support and administrative tasks. The Care Team Lead will be adaptable, knowledgeable, empathetic, and organized, committed to excellence in home care delivery.

Essential Duties:  

·      Introduce new caregivers to clients, train and coach new caregivers in shadow shifts to ensure a smooth transition

·      Perform all duties expected of a Direct Caregiver during direct care

·      Conduct scheduled check-ins to assess care quality and satisfaction, ensuring compliance with care standards and protocols

·      Monitor and support care plan implementation, communicating with Client Care Coordinator and Scheduling as needed as client needs evolve

·      Provide real-time feedback, coaching, and on-the-spot training during visits

·      Maintain understanding of care plans, client schedules and operational needs

·      Cover open shifts as needed or directed by management

·      Provide input for caregiver performance evaluations and development

·      Support continuous education and training by assisting with in-person training alongside Admin Team

·      Complete required documentation accurately and timely

·      Support admin team as needed

·      Additional duties and responsibilities as needed assigned by management

Essential Areas of Responsibility:  

·      Ensure caregivers follow procedures, especially for specialized care (dementia, mobility, medical equipment)

·      Identify and report issues or deviations from care plans to management

·      Promote excellence, compassion, and professionalism in all services

·      Provide direct care as needed

Knowledge, Skills, and Experience Requirements:  

·      At least one year’s experience in leading and coaching others

·      3+ years in as a caregiver (CNA preferred)

·      Natural leader with strong mentoring abilities

·      Physically able to perform caregiving duties

·      Compassionate, ethical, dependable and professional

·      Knowledgeable in using medical equipment to transfer clients

·      Willing to do personal cares for female and male clients

·      Background in long-term care, assisted living, dementia care, nursing homes and hospice is highly desirable

·      Familiarity with home care practices, client rights

·      Valid MN Driver’s License and valid proof of insurance

Physical Demands:  

·      Ability to successfully operate various types of office equipment

·      Must be able to work at desk/computer for various lengths of time

·      Walking, standing, sitting for various lengths of time

·      Ability to lift up to 50 lbs. as needed

·      Pushing and/or pulling

·      Bending, twisting and reaching

·      Eye / hand coordination, ability to read, write and communicate effectively
