Registered Nurse Case Manager

Chapters Health System is seeking an RN Case Manager to provide direct patient care in a home-based setting, coordinating the Plan of Care with the IDT and educating families on disease progression and comfort measures. The role supports keeping patients in their homes whenever possible. The position requires active RN licensure in the working state, at least one year of nursing experience with hospice or hospital settings preferred, and a valid BLS certification. #J-18808-Ljbffr
