Registered Nurse Case Manager

Chapters Health System is seeking an RN Case Manager to provide direct patient care in a home-based setting, coordinating the Plan of Care with the IDT and educating families on disease progression and comfort measures. The role supports keeping patients in their homes whenever possible.
