Job Description Requirements:

• Master's degree in Social Work (MSW)
• Active Florida LCSW license (required)
• Minimum 1 year of recent acute care hospital social work experience OR 5+ years of prior hospital social work experience within the last 3–4 years
• Mandatory discharge planning experience
• Experience with PASSRs and ESPERs
• LTAC or Hospice experience accepted if combined with hospital experience
• Strong multitasking, organizational, and critical thinking skills

Job Description:

The Social Worker (MSW) provides comprehensive biopsychosocial support for inpatient units and the emergency department. This role focuses on crisis intervention, discharge planning, high-risk patient support, and coordination of post-acute resources. The Social Worker collaborates closely with case management, physicians, nursing staff, and external agencies to remove barriers to care, support patient self-determination, and ensure safe, effective transitions of care across multiple service lines, including ICU, Trauma, Oncology, NICU, Transplant, and ED.

Unit Details:

The Social Worker conducts psychosocial assessments, identifies emotional, financial, and social barriers to discharge, and develops individualized discharge plans for high-risk patients. Responsibilities include coordinating post-acute placements, facilitating family and care-team meetings, completing crisis interventions, and providing emotional support during end-of-life or critical events. The role requires ongoing collaboration with interdisciplinary teams, insurance providers, and community resources, as well as maintaining knowledge of Medicare, Medicaid, and managed care regulations to support appropriate length-of-stay and resource utilization.
