Licensed Practical Nurse (LPN) - Canton, Maine Job Summary Pinnacle Health & Rehab at Canton is seeking a dynamic and highly motivated Licensed Practical Nurse (LPN) to join our clinical team. As a Licensed Practical Nurse, you will be responsible for providing high‑quality patient care in a 66‑bed Assisted Living facility. Responsibilities Provide direct patient care under the supervision of a Registered Nurse (RN) or other licensed healthcare providers Lead and oversee a clinical team, working collaboratively with direct care workers and ancillary departments Develop and implement individualized care plans for residents, ensuring a high level of customer satisfaction Monitor and report changes in residents' conditions to the RN or other licensed healthcare providers Participate in quality improvement initiatives and maintain accurate and up‑to‑date records Collaborate with other healthcare professionals to ensure seamless continuity of care Requirements Active Maine nursing license Proof of MMR, varicella, Hep B, and seasonal flu vaccination (Oct‑Apr) Ability to work every other weekend with on‑call responsibilities Experience in Assisted Living Care is a plus, but not required.
