Role Overview A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence, following physician orders and plans of care under the supervision of the Director of Clinical Services/Clinical Manager. Care is rendered in compliance with current federal, state, and local standards and regulatory requirements, and as directed by Clinical Manager, Director of Nursing, or Administrator to maintain the highest quality care. Responsibilities

• Makes initial evaluation and regularly reevaluates patient nursing needs.
• Develops, initiates, and revises the Plan of Care (POC) as necessary to ensure quality and continuity of care.
• Provides skilled nursing services requiring specialized nursing skills, including preventive and rehabilitative nursing procedures.
• Prepares clinical and progress notes for each patient visit and summaries of care conferences in a timely manner as per agency policy.
• Coordinates services and informs personnel of changes in the condition and needs of the patient.
• Counsels patient and family/significant others in meeting nursing and related needs.
• Participates in and presents at in‑service programs.
• Applies infection control plan and universal precautions to protect patient and staff (OSHA).
• Processes orders, notifies physician of patient needs and condition changes, and completes certification/recertification orders and discharge summaries.
• Determines the amount and type of nursing needed by each patient and refers to PT, SLP, OT, and Medical Social Worker as required.
• Develops Home Health Aide Care Plan and performs supervisory visits for HHA and LPN/LVN.
• Provides total patient care as needed and performs on‑call duty nights, weekends, and holidays as assigned.
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition per agency defined time frames.
• Utilizes ICD‑10 codes appropriately.

Qualifications RN licensure required. Bachelor’s degree or equivalent. Less than 5 years of technical experience. Work Schedule & Compensation Weekly Hours: 1 (variable schedule). Pay Range: $77,200 - $106,200 per year (starting base pay).
