You will utilize telehealth technology to connect with clients, enabling you to conduct meaningful sessions that promote their mental well-being. Key Responsibilities\n \n Conduct comprehensive mental health assessments, evaluating both psychological and medical conditions that might affect patients’ mental health. \n Develop and implement personalized treatment plans that address the diverse needs of your clients, ensuring a holistic approach to care. \n Facilitate engaging individual and group therapy sessions utilizing evidence-based methodologies designed to foster growth and resilience. \n Work collaboratively with healthcare providers to create coordinated care pathways that enhance treatment outcomes for patients. \n Provide immediate crisis intervention support for clients in urgent mental health situations, applying quick-thinking skills and compassion. \n Educate patients on the interaction between mental health and physical conditions, empowering them to take charge of their overall wellness. \n Ensure all patient records and documentation are meticulously maintained in compliance with legal, ethical, and organizational standards. \n Engage actively in team meetings and case conferences to share insights and refine care strategies across disciplines. \n Stay informed about the latest developments in integrated care practices and contribute to continuous improvement efforts. \n Patient Population / Client Focus\n You will be working with a diverse patient population, including individuals experiencing a variety of mental health disorders alongside physical health issues. Your role is critical in bridging the gap between mental and physical health, supporting patients through their unique challenges and helping them achieve overall well-being. Qualifications\n \n A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution. \n Current and valid licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), or equivalent qualifications in the state of New York. \n Proven experience in addressing mental health needs in patients with co-occurring medical conditions. \n Exceptional interpersonal and communication skills, enabling effective rapport-building with patients. \n Ability to maintain professional boundaries while ensuring adherence to ethical guidelines and patient confidentiality. \n Preferred Experience\n \n Familiarity with electronic health record systems and telehealth technology. \n Experience in team-based care settings, contributing to multidisciplinary treatment strategies. \n Demonstrated commitment to professional development and lifelong learning in the field of mental health. \n Compensation and Benefits\n In recognition of your contributions to our mission, the salary for this position ranges from USD 80,000 to 100,000 annually.
